What should you bring with you to your first Physical Therapy appointment?

1. Completed New Patient Forms (see Forms above)
2. Completed Functional Questionnaire(s) (See Forms above)
3. Physical Therapy Prescription/Referral (if applicable)
4. Picture ID (i.e. Driver's License)
5. Workout attire and footwear is ideal
6. Post-operative Therapy Protocol (if given one by your surgeon)

How much time will Dr. Winters spend with you for each Physical Therapy appointment?

Whether it’s your first appointment, or a follow-up visit, every session will be 45-60 minutes. Dr. Winters is the owner of the practice and the sole provider. Accordingly, every session will be one-on-one, and 100% with him. This enables him to be as thorough, efficient, and effective as possible, as he takes an individualized approach with EVERY patient.
Do you need a Prescription from your Doctor to attend Physical Therapy?
No. Given that Dr. Winters has Direct Access Authorization, he has the ability to evaluate and treat patients for up to 30 days WITHOUT a prescription. However, if deemed necessary to continue therapy after 30 days, you would need a script to do so.
Does Independence Spine & Orthopedic accept your insurance?
Given that Dr. Winters is an out-of-network provider, he does not participate in insurance plans.
Can you submit a claim to your insurance company for reimbursement?
Yes. If you want to submit a claim, you can receive a “Superbill” via email - which is an itemized bill for each session. There is no guarantee your insurance company will reimburse you for physical therapy services, thus Dr. Winters HIGHLY RECOMMENDS you contact your insurance company prior to your first visit to verify your out-of-network benefits.
What are acceptable forms of payment for Physical Therapy services at Independence Spine & Orthopedic?
Cash, check, and/or credit cards are all acceptable forms of payment for Physical Therapy services.
Does Independence Spine & Orthopedic accept Medicare patients?
Given that Dr. Winters is an out-of-network provider, he is unable to provide physical therapy services to Medicare beneficiaries.
